PLEASE READ STATEMENT BEFORE REGISTERING FOR CLASSES:
Bloom Recovery Network will no longer hold virtual DIP Programming. This option was only set in place amidst the State of Emergency surrounding the COVID 19 Pandemic. We will now hold in-person only classes.
Bloom Recovery Network offers OHMAS Certified 72 Hour Driver's Intervention Program. Our Toledo DIP Programs are held at the Quality Inn Perrysburg located at 10621 Fremont Pike in Perrysburg, OH 43551. Our DIP begins @ 4p Thursday and ends @ 4p Sunday on each date listed below. A deposit of $50 is due at time of registration. The remaining balance must be paid in full at least one week prior to program start date.NO CHANGES TO ROSTER WILL BE MADE AFTER 4PM ON THE THURSDAY PRIOR TO THE EVENT START DATE. ALL MONIES PAID WILL BE LOST IF YOU DO NOT ATTEND ONCE THAT DEADLINE HAS PASSED.

Printable Forms:
In order to register, you must submit the Intake and Authorization for Release of Information forms along with a $50 deposit. Final payment will be due one week prior to the event you wish to attend.
Note: You will not be contacted upon receipt of registration documents. You will receive a reminder letter closer to your program start date. The courts, however, will be notified immediately upon receipt. Approved payment methods: PAYPAL, Cash, Check, Money Order, Credit Card (processing fee attached)
