LOCATION:  Our Toledo DIP Programs are held at the Quality Inn Perrysburg located at 10621 Fremont Pike in Perrysburg, OH 43551. 

SCHEDULE: Our DIP begins at 4pm Thursday and ends at 4pm Sunday. 

COST:  SINGLE ROOM $600 DOUBLE ROOM $475 

DEPOSIT: A deposit of $50 is due at time of registration. 

The remaining balance must be paid in full at least one week prior to program start date.

NO CHANGES TO THE ROSTER WILL BE MADE AFTER 4PM ON THE THURSDAY PRIOR TO THE EVENT START DATE. ALL MONIES PAID WILL BE LOST IF YOU DO NOT ATTEND ONCE THAT DEADLINE HAS PASSED​​. 

*Some local courts offer financial assistance. If your court has offered this support, please email amityparlapiano.bloomrecovery@gmail.com after you complete the steps outlined above. Please note: Unless the court is paying the entire amount, a deposit is still required at the time of registration*

You must complete all Registration Forms via the link below; pay a deposit or full payment when prompted at the end of the form. 

YOU ARE NOT CONSIDERED REGISTERED UNTIL THE DEPOSIT IS PAID. ONCE PAYMENT AND COMPLETED FORMS ARE RECEIVED, WE WILL CONTACT THE COURTS. 

PLEASE NOTE: While your courts are contacted immediately upon registration, you will not hear from us until closer to your program start date.

 ADULT REMEDIAL COURSE: Bloom Recovery no longer facilitates the Adult Remedial Course due to it's online availability. Please CLICK on the button below to find a provider.  

To register, you must submit the Intake and Authorization for Release of Information forms along with a $50 deposit (see printable forms below). Final payment will be due one week before the event you wish to attend. 

Note:  You will not be contacted upon receipt of registration documents.  You will receive a reminder letter closer to your program start date.  The courts, however, will be notified immediately upon receipt. Approved payment methods: PAYPAL, Cash, Check, Money Order, Credit Card (processing fee attached)
